Your First Visit
To allow time to complete new patient information forms, please arrive 15-minutes early. Bring your insurance card, names of current medications and medical history information. Lakeshore Health Partners participates with most insurance carriers, however, you should confirm coverage and eligibility with your carrier prior to your first appointment.
If your HMO or PPO requires you to select a primary care provider, be sure one of our physicians’ names is listed as your provider. If your carrier requires a referral when seeing a specialist, please obtain the necessary referral or call our office for assistance.

If you are unable to keep your appointment or will be late, please call us. We will gladly reschedule your visit. Patients arriving late for an appointment may be asked to reschedule. Please make every effort to keep your appointment.
Office Hours
General office hours are Monday through Friday,
8:00 a.m. - 5:00 p.m.
Several of our locations offer extended evening and Saturday hours. Call for more information.
After-Hours Care/Emergencies
For medical urgencies occurring after hours, call our office to be connected to our answering service for further instructions. For all life-threatening emergencies, always proceed directly to the nearest emergency room. Walk-in Care.
Prescription Refills
If you require a refill between appointments, please call during regular office hours. To make renewing your prescription over the phone as convenient as possible, follow these guidelines:
- Provide your name & phone number, the medication name & the number of your preferred pharmacy.
- Give us at least 24—48 hours to renew your prescription.
- If you have not been seen in our office, we will not be able to refill your prescription over the phone. Contact our office to schedule an appointment.
- Plan ahead. Medications will not be refilled evenings or weekends except in emergencies.
Test Results
We understand that patients are eager to hear the status of their lab, X-ray or other test results. You can expect to hear from us in person, or by mail or phone. If you have not received notice of your test results within two weeks, please contact our office.
Insurance and Billing
As a courtesy, we bill insurance carriers directly but require payments for deductibles and non-covered services at the time of service. We accept cash, checks and major credit/debit cards. For questions about your specific insurance coverage, contact your insurance carrier directly.

Confidentiality
We will not release information about your care without your written authorization. All staff follow strict policies and procedures to ensure confidentiality and to protect your personal privacy and protected health information (PHI). Â
Advanced Directives
All adults are encouraged to sign an Advanced Directive. An Advanced Directive enables you to assign a person to make medical decisions for you should you become unable to make them yourself. Talk to our office staff to learn more about Advanced Directives
